Meals on Wheels CORI Information
Thank you for your interest and support for Meals on Wheels. If you are interested in driving on a regular basis, occasional basis or as a back-up driver, you will need to complete the attached Volunteer application and CORI form and provide a copy of your license. Once your paperwork is complete, I will contact you regarding driving opportunities.
Mailing in your Application
- Print and complete the attached application and CORI form.
- You do not need to complete the 5 questions on the middle of the volunteer application page.
- Print a copy of your license.
- Please indicate on the application that you are with LexFUN!
- Return to the address below:

Online Applications
- Open the Volunteer Application and CORI Application and complete online.
- You do not need to complete the 5 questions on the middle of the volunteer application page.
- Save the documents by your last name (i.e. Smith Volunteer Application and Smith CORI Form)
- If possible, please send a scanned copy of your license. If you cannot scan your license, please send a photocopy by mail.
- Attach these documents to an email
- Please indicate in your email that you are with LexFUN!
